Introducción: El carcinoma basoescamoso es un subtipo agresivo de carcinoma basocelular compuesto por células basaloides y áreas de células escamosas con una zona de transición intermedia, con tendencia a la recurrencia y metástasis. Objetivo: Describir el caso clínico de una paciente con un carcinoma basoescamoso en región temporal cerca del canto externo del ojo izquierdo. Presentación de caso: Se presentó el caso de una paciente con un carcinoma basoescamoso en región temporal cerca del canto externo del ojo izquierdo de 30 mm de diámetro. Se aplicó HeberFERON con respuesta completa al eliminar el tumor. Conclusiones: El HeberFERON es una opción no quirúrgica de tratamiento que puede ser usada en el carcinoma basoescamoso de localización facial que por su tamaño puede provocar mutilaciones o deformidades en esta zona(AU)


Introduction: Basal squamous cell carcinoma is an aggressive subtype of basal cell carcinoma composed of basaloid cells and areas of squamous cells with an intermediate transition zone, with a tendency to recur and metastasize. Objective: To describe the clinical case of a patient with a basal squamous cell carcinoma in the temporal region near the external canthus of the left eye. Case report: This paper reports a case of a female patient with a basal squamous cell carcinoma in the temporal region near the external canthus of her left eye with 30 mm diameter. HeberFERON was used with complete response when eliminating the tumor. Conclusions: HeberFERON is a non-surgical treatment option that can be used in facial basal squamous cell carcinoma that, due to its size, can cause mutilations or deformities in this area(AU)

Introducción: El aumento de la resistencia a los antimicrobianos constituye actualmente una peligrosa amenaza para la salud. Ante este problema global de falta de antimicrobianos, es perentorio intervenir de forma coordinada e idear fórmulas para incentivar la investigación a nivel internacional. Objetivo: Realizar una revisión actualizada sobre las causas y mecanismos de la resistencia a los antibióticos y la adaptación del sistema CRISPR/Cas para el desarrollo de innovadores antimicrobianos como parte esencial de una estrategia altamente específica en el tratamiento de infecciones producidas por bacterias resistentes. Métodos: Se realizó una revisión documental, se empleó la bibliografía nacional e internacional especializada publicada en los últimos 5 años. Se utilizó el motor de búsqueda Google Académico y se consultaron artículos de libre acceso en las bases de datos Pubmed, SciELO, LILACS, CUMED y HINARI, en el período comprendido entre marzo de 2020 hasta el mes de enero de 2021. Se revisaron un total de 41 artículos. Las consultas se hicieron en inglés y español. Para la búsqueda se tuvo en cuenta las palabras clave: eligobióticos; resistencia a antibióticos; CRISPR/Cas. Resultados: La evidencia recopilada sustenta que muchas enfermedades son inducidas por alteraciones del equilibrio de la microbiota humana y la técnica de edición genética CRISPR/Cas permitirá el desarrollo de novedosos antibióticos como los eligobióticos que eliminarán las bacterias patógenas multirresistentes y dejarán intacto el microbioma. Conclusiones: el esclarecimiento de los enigmas de la microbiota y su diseño con terapia génica permitirán el progreso de innovadores antibióticos con empleo del sistema CRISPR/Cas que ineludiblemente modificarán la práctica médica para siempre(AU)


Introduction: The increase in antimicrobial resistance is currently a dangerous threat to health. Faced with this global problem of lack of antimicrobials, it is imperative to intervene in a coordinated manner and devise formulas to encourage research at the international level. Objective: To review on the update causes and mechanisms of antibiotic resistance and the adaptation of CRISPR/Cas system for the development of innovative antimicrobials as an essential part of a highly specific strategy in the treatment of infections caused by resistant bacteria. Methods: A documentary review was carried out in the specialized national and international bibliography published in the last 5 years. Google Scholar search engine was used and free access articles were consulted in Pubmed, SciELO, LILACS, CUMED and HINARI databases, from March 2020 to January 2021. A total of 41 articles were retrieved. The consultations were made in English and Spanish. For the search, we took into account the keywords eligobiotics, antibiotic resistance, CRISPR/Cas. Results: The reviewed evidence supports that many diseases are induced by alterations in the balance of the human microbiota; and CRISPR/Cas gene editing technique will allow the development of novel antibiotics such as eligobiotics that will eliminate multi-resistant pathogenic bacteria and leave the microbiome intact. Conclusions: The clarification of the enigmas of the microbiota and its design with gene therapy will allow the progress of innovative antibiotics using CRISPR/Cas system that will inevitably change medical practice forever(AU)

Se presentan los resultados del estudio de intervención realizado en el Centro Nacional de Biopreparados, empresa de BioCubaFarma. La investigación fue promovida por el Instituto Finlay de Vacunas, con el objetivo de evaluar los efectos directos e indirectos de la vacunación anti SARS-CoV-2 con un esquema heterólogo 2P+1: dos dosis de SOBERANA®02 más una dosis de SOBERANA®Plus con 28 días entre ellas, en cohortes poblacionales de riesgo de infección, enfermedad y dispersión de la epidemia. Fueron evaluados 1.007 sujetos, incluyéndose inicialmente 924. De ellos, el 97,62 por ciento recibió el esquema completo de vacunación. Posteriormente se incluyeron 21 convalecientes de COVID-19 con al menos dos meses del alta clínica, que recibieron una única dosis de SOBERANA®Plus. La seguridad de las vacunas se evalúo mediante la identificación y clasificación de los eventos adversos por farmacovigilancia activa y pasiva. Se registraron un total de 482 eventos adversos, en su mayoría por farmacovigilancia pasiva, de intensidad leve y de causalidad A1 (relacionados). No ocurrieron eventos adversos graves relacionados. Se realizó cuantificación de IgG anti SARS-CoV-2 a 100 individuos y el 68 por ciento tuvo una respuesta mayor o igual de 50 UA/mL, siendo estos sujetos nueve años menores que los de respuesta menor. Hasta los tres meses de concluida la intervención, 64 vacunados fueron diagnosticados con COVID-19 y ninguno de ellos estuvo grave o falleció. Se evidenció un perfil de seguridad muy favorable de SOBERANA® e indicios de efectividad en la prevención de formas graves y mortalidad por COVID-19(AU)


The results of an intervention study performanced at the Centro Nacional de Biopreparados, a company of BioCubaFarma, are presented. The research was promoted by the Finlay Vaccine Institute, with the aim of evaluating the direct and indirect effects of vaccination against SARS-CoV-2, with a heterologous 2P + 1 scheme: two doses of SOBERANA®02 plus one dose of SOBERANA®Plus with 28 days between them; in population cohorts at risk of infection, disease and spread of the epidemic. A quantity of 1,007 subjects were evaluated and 924 were initially included. Of these, 97.62 percent received the complete vaccination schedule. Subsequently, 21 convalescents of COVID-19 with at least two months of clinical discharge were included, who received a single dose of SOBERANA®Plus. The safety of the vaccine was evaluated by identifying and classifying adverse events by active and passive pharmacovigilance. A total of 482 adverse events were recorded, mostly due to passive pharmacovigilance, mild intensity and A1 causality (related). No related serious adverse events occurred. IgG anti SARS-CoV-2 was quantified in 100 individuals, and 68 percent had a response greater than or equal to 50 IU/mL, these subjects being nine years younger than those with a lower response. Up to three months after the intervention, 64 vaccinated people were diagnosed with COVID-19 and none of them were seriously ill or died. A very favorable safety profile of SOBERANA® and indications of effectiveness in preventing severe forms and mortality from COVID-19 were evidenced(AU)

Introducción: El pie diabético es una alteración clínica de base etiopatogénica neuropática e inducida por la hiperglucemia mantenida, en la que con o sin coexistencia de isquemia, y previo desencadenante traumático, produce lesión y/o ulceración del pie. Objetivo: Describir los resultados del tratamiento con Heberprot-P® en pacientes con pie diabético. Métodos: Estudio de corte transversal descriptivo y prospectivo en pacientes con pie diabético pertenecientes al policlínico Joaquín de Agüero y Agüero del municipio Camagüey, durante el período de julio del 2019 a julio del 2020. El universo de estudio estuvo integrado por un total de 60 pacientes con diagnóstico de pie diabético a quienes se les aplicó factor de crecimiento epidérmico. La información se procesó a través del paquete estadístico SPSS versión 21. Resultados: Predominaron los pacientes del grupo de edad de 65 a 74 años (35 por ciento), el sexo femenino (58,8 por ciento), con hipertensión arterial asociada (81,8 por ciento). El 38,4 por ciento de los diabéticos se presentaron con lesiones en los dedos, grado II (63,3 por ciento) según la escala de Wagner. Conclusiones: Con la aplicación del Heberprot-P® presentaron dolor (78,3 por ciento) como efecto adverso y una cicatrización total a la respuesta del tratamiento (96,6 por ciento), lográndose mantener la integridad del miembro(AU)


Introduction: Diabetic foot is a clinical alteration of neuropathic etiopathogenic basis, induced by maintained hyperglycemia, in which, with or without coexistence of ischemia, as well as previous traumatic trigger, foot injury or ulceration is produced. Objective: To describe the outcomes of treatment with Heberprot-P® in patients with diabetic foot. Methods: Descriptive and prospective cross-sectional study carried out in patients with diabetic foot belonging to the Joaquín de Agüero y Agüero Polyclinic of Camagüey Municipality, during the period from July 2019 to July 2020. The study universe was made up of a total of sixty patients diagnosed with diabetic foot who were applied epidermal growth factor. The information was processed through the statistical package SPSS version 21. Results: Patients in the 65-74 years age group predominated (35 percent), together with the female sex (58.8 percent), as well as associated arterial hypertension (81.8 percent). 38.4 percent of diabetic patients presented with finger lesions of grade II (63.3 percent) according to the Wagner's classification. Conclusions: With the application of Heberprot-P®, pain (78.3 percent) appeared as an adverse effect. There was also total healing as a response to treatment (96.6 percent), which permitted to maintain limb integrity(AU)

O tratamento do angioedema hereditário tem início com a educação dos pacientes e familiares sobre a doença, pois é fundamental o conhecimento da imprevisibilidade das crises, assim como os seus fatores desencadeantes. O tratamento medicamentoso se divide em terapia das crises e profilaxia das manifestações clínicas. As crises devem ser tratadas o mais precocemente possível com o uso do antagonista do receptor de bradicinina, o icatibanto ou o concentrado de C1-inibidor. É necessário estabeler um plano de ação em caso de crises para todos os pacientes. A profilaxia de longo prazo dos sintomas deve ser realizada preferencialmente com medicamentos de primeira linha, como concentrado do C1-inibidor ou o anticorpo monoclonal anti-calicreína, lanadelumabe. Como segunda linha de tratamento temos os andrógenos atenuados. Na profilaxia de curto prazo, antes de procedimentos que podem desencadear crises, o uso do concentrado de C1-inibidor é preconizado. Existem algumas restrições para uso desses tratamentos em crianças e gestantes que devem ser consideradas. Novos medicamentos baseados nos avanços do conhecimento da fisiopatologia do angioedema hereditário estão em desenvolvimento, devendo melhorar a qualidade de vida dos pacientes. O uso de ferramentas padronizadas para monitorização da qualidade de vida, do controle e da atividade da doença são fundamentais no acompanhamento destes pacientes. A criação de associações de pacientes e familiares de pacientes com angioedema hereditário tem desempenhado um papel muito importante no cuidado destes pacientes no nosso país.


The treatment of hereditary angioedema begins with the education of patients and their families about the disease, as it is essential to know the unpredictability of attacks as well as their triggering factors. Drug treatment is divided into attack therapy and prophylaxis of clinical manifestations. Attacks should be treated as early as possible with the bradykinin receptor antagonist icatibant or C1-inhibitor concentrate. An action plan needs to be established for all patients with attacks. Long-term prophylaxis of symptoms should preferably be performed with first-line drugs such as C1-inhibitor concentrate or the anti-kallikrein monoclonal antibody lanadelumab. Attenuated androgens are the second line of treatment. In short-term prophylaxis, before procedures that can trigger attacks, the use of C1-inhibitor concentrate is recommended. There are some restrictions for the use of these treatments in children and pregnant women that should be considered. New drugs based on advances in knowledge of the pathophysiology of hereditary angioedema are under development and are expected to improve patient quality of life. The use of standardized tools for monitoring quality of life and controlling disease activity is essential in the follow-up of these patients. The creation of associations of patients and families of patients with hereditary angioedema has played a very important role in the care of these patients in Brazil.

Introducción: Recientemente se han descubierto nuevos medicamentos para el tratamiento de la diabetes tipo 2, con novedosos mecanismos de acción y menos efectos adversos. Dentro de ellos tenemos los análogos del péptido similar al glucagón tipo 1. Objetivo: Explicar la evidencia existente sobre los efectos del tratamiento con agonistas del receptor del péptido similar al glucagón tipo 1 en las personas con obesidad y diabetes mellitus tipo 2. Material y Métodos: Se realizó una revisión sistemática que incluyó estudios de los efectos de los agonistas del receptor del péptido similar al glucagón tipo1 como tratamiento en personas mayores de 12 años con obesidad y diabetes tipo 2. Se realizó una síntesis narrativa formal de los datos recogidos, no se realizó una síntesis estadístico formal. La calidad de evidencia para cada desenlace se determinó, según la metodología Grading of Recommendations Assessmet, Developmet and Evaluation. Resultados: La evidencia disponible demuestra que los agonistas del receptor del péptido similar al glucagón tipo 1, lograron una mayor disminución del peso corporal (-7,0 por ciento vs -2 por ciento) y de las cifras de hemoglobina glucosilada (HbA1c) (-0,40 por ciento vs -0,10 %) respecto al grupo placebo. Además, de una mayor reducción de la cintura abdominal. Conclusiones: La evidencia analizada muestra que los fármacos del tipo agonistas del receptor del péptido similar al glucagón tipo 1 tienen efectos beneficiosos en el tratamiento de las personas con obesidad y diabetes, disminuyendo el peso corporal y los valores de glucemia(AU)


Introduction: New drugs with novel mechanisms of action and fewer adverse effects have recently been discovered for the treatment of type 2 diabetes. Among them are glucagon-like peptide-1 analogues. Objective: To explain the existing evidence of the effects of treatment with glucagon-like peptide-1 receptor agonists in people with obesity and type 2 diabetes mellitus. Material and Methods: We conducted a systematic review that included studies on the effects of glucagon-like peptide -1 receptor agonists for the treatment of people older than 12 years with obesity and type 2 diabetes. A formal narrative synthesis of the collected data was performed, whereas a formal statistical synthesis was not performed. The quality of evidence for each outcome was determined according to the Grading of Recommendations Assessment, Development and Evaluation method. Results: The available evidence shows that glucagon-like peptide -1 receptor agonists achieved a greater reduction in body weight (-7,0 percent vs -2 percent) and glycosylated hemoglobin (HbA1c) (-0,40 percent vs -0,10 percent) compared to the placebo group. In addition, there was a greater reduction in abdominal waist circumference. Conclusions: The evidence analyzed shows that glucagon-like peptide -1 receptor agonists have beneficial effects in the treatment of people with obesity and diabetes, reducing body weight and glycemia values(AU)

Introdução: Caryocar brasiliense é conhecida popularmente como Pequi ou Pequizeiro, sendo uma planta com propriedades medicinais para tratamento de doenças respiratórias, úlceras gástricas e dores musculares. Objetivo: comparar a extração de biocompostos por meio de dois métodos extrativos, agitação magnética e banho ultrassônico. Além disso, avaliar a atividade antioxidante, o teor de compostos fenólicos, flavonoides e antocianinas em extratos das folhas e da casca de Pequi (C. brasiliense), com vistas a agregar valor quanto às suas propriedades funcionais. Metodologia: o conteúdo de compostos fenólicos foi determinado pelo método de Folin-Ciocalteu, flavonoides e antocianinas pelo método de Lima e Melo. A atividade antioxidante foi medida pelo método 2,2-difenil-1-picrilhidrazil (DPPH). Resultados: o extrato das folhas de Pequi exibiu maior teor de fenólicos em relação à casca, independente do método de extração. O extrato das folhas obtido em banho ultrassônico apresentou forte atividade antioxidante com valor de 72,2%. Conclusão: os extratos de Pequi demonstraram um perfil fitoquímico promissor que deve ser investigado no futuro para aplicação farmacológica como adjuvante ou precursor na síntese de novos cosméticos ou medicamentos com propriedades antioxidante.


Introduction: Caryocar brasiliense is a medicinal plant used in the treatment of respiratory diseases, gastric ulcers and muscle pain. Objective: to compare the extraction of natural compounds using two extractive methods, magnetic stirring and ultrasonic bath. In addition, to evaluate the antioxidant activity, the content of phenolic compounds, flavonoids and anthocyanins in the leaves and bark of Pequi (C. brasiliense), with a view to adding value through its functional properties. Methodology: the phenolic content was determined by the Folin-Ciocalteu method, flavonoids and anthocyanins by the Lima and Melo method. Antioxidant activity was measured using the 2.2-diphenyl-1-picrilhhydrazyl (DPPH) method. Results: the extract of the Pequi leaves exhibited a higher phenolic content in relation to the bark for both extraction methods. The Pequi leaf in an ultrasonic bath showed strong antioxidant activity with a value of 72.2%. Conclusion: Pequi extracts demonstrated a promising phytochemical profile that should be investigated in the future for pharmacological application as an adjuvant or precursor in the synthesis of a new cosmetic or medicine with antioxidant function.

In Egypt, the lyophilized live attenuated sheep pox virus vaccine has been used for the vaccination of cattle against lumpy skin disease virus to control its economic impact on livestock industry. In this endeavor, we validate the efficacy of Carbopol® as a stabilizer and adjuvant to enhance immunogenicity of such a heterologous sheep pox virus vaccine against lumpy skin disease. Lyophilization of sheep pox virus vaccine stabilized with Carbopol® produced better physical and antigenic properties than freeze-drying with lactalbumin/sucrose stabilizer; this was manifested by superior disc uniformity, thermo-stability at 37oC, and less reduction in virus titer. Immunization of calves' groups with variable sheep pox vaccine doses containing different Carbopol® concentrations revealed that 103.5 TCID50 of sheep pox virus vaccine enclosing 0.5 percent Carbopol® is the field dose of choice. Moreover, it induced protective serum neutralizing index of 2.5 and a ELISA S/P ratio of 36, by the 4th week post vaccination. Besides, the inclusion of 0.5 percent Carbopol® in formulation of the sheep pox virus vaccine was safe in bovines and enhanced cellular immune response to lumpy skin disease virus, as evidenced by increased T cell proliferation. Hence, it is recommended to use Carbopol® as 0.5 percent in preparation of live attenuated sheep pox virus vaccine to confer better protection against lumpy skin disease virus infection(AU)


En Egipto, la vacuna atenuada liofilizada contra el virus de la viruela ovina ha sido utilizado para la vacunación del ganado, contra el virus de la dermatosis nodular contagiosa, para controlar su impacto económico en la industria ganadera. En este trabajo, validamos la eficacia del Carbopol®, como estabilizador y adyuvante, para mejorar la inmunogenicidad de dicha vacuna heteróloga contra la dermatosis nodular contagiosa. La liofilización de la vacuna contra el virus de la viruela ovina estabilizada con Carbopol®, resultó en mejores propiedades físicas y antigénicas que la liofilización con el estabilizador de lactoalbúmina/sacarosa; lo anterior se manifestó en la uniformidad superior del disco, la termoestabilidad a 37°C y la menor reducción del título del virus. La inmunización de grupos de terneros con dosis variables de vacuna contra el virus de la viruela ovina, que contenían diferentes concentraciones de Carbopol®, reveló que la dosis de campo de elección fue 103,5 TCID50 de la vacuna contra el virus de la viruela ovina conteniendo 0,5 por ciento de Carbopol®, la que indujo un índice de neutralización sérica protectora de 2,5 y una relación S/P de ELISA de 36 a la cuarta semana después de la vacunación. Además, la inclusión de Carbopol® al 0,5 por ciento en la formulación de la vacuna contra el virus de la viruela ovina fue segura en los bovinos y potenció la respuesta inmunitaria celular contra el virus de la dermatosis nodular contagiosa, como lo demuestra el aumento de la proliferación de células T. Por lo tanto, se recomienda el uso de Carbopol® al 0,5 por ciento en la preparación de la vacuna viva atenuada contra el virus de la viruela ovina para conferir una mejor protección contra la infección por el virus de la dermatosis nodular contagiosa(AU)

Haemophilus influenzae tipo b es un importante patógeno del hombre causante de varias de las enfermedades invasivas en niños menores de cinco años, contra el cual fueron autorizadas las vacunas glicoconjugadas a partir del polirribosilribitol fosfato. Quimi-Hib® es la primera y única vacuna contra este patógeno que utiliza el polisacárido obtenido por síntesis química. El Ingrediente Farmacéutico Activo es producido por el Centro de Ingeniería Genética y Biotecnología y se obtiene a partir de su conjugación al toxoide tetánico. En el presente reporte se hizo una caracterización del polirribosilribitol fosfato mediante la técnica de cromatografía de exclusión molecular de alta eficacia con detección ultravioleta a 215 nm. En el estudio se evaluaron tres lotes y se determinó el perfil de elución en una columna SuperdexTM 75 10/300 GL Increase con un porciento de pureza de 77,42 ± 8,97 y una masa molar promedio de 7.381 Da ± 210,93. La principal impureza presente en el polirribosilribitol fosfato es el dimetilsulfóxido, disolvente utilizado en la reacción de activación con el éster N-hidroxisuccinimidilo del ácido β-maleimidopropiónico. El polirribosilribitol fosfato se purificó por filtración con un Amicon Ultra-15 de 2.000 Da hasta una pureza de 99,1 por ciento y se conjugó al toxoide tetánico. El rendimiento de la reacción de conjugación con el polisacárido purificado fue de 30,0 por ciento 1,77 el cual no muestra diferencias significativas con el control que fue 33,7 por ciento ± 3,57 demostrándose que el dimetilsulfóxido no afecta el desempeño de la reacción de conjugación(AU)


Haemophilus influenzae type b is an important human pathogen causing some invasive diseases in children less than five years of age. Glycoconjugate vaccines based on polyribosylribitol phosphate have been licensed against this bacterium. Quimi-Hib® is the first and only vaccine against this pathogen using the chemically synthesized polysaccharide. The Active Pharmaceutical Ingredient is produced by the Center for Genetic Engineering and Biotechnology and is obtained from its conjugation to tetanus toxoid. In the present report a characterization of polyribosylribitol phosphate was performed by high performance molecular exclusion chromatography with ultraviolet detection at 215 nm. Three batches were evaluated in the study and the elution profile was determined on a SuperdexTM 75 10/300 GL Increase column with a purity percentage of 77.42 ± 8.97 and an average molecular weight of 7,381 Da ± 210.93. The main impurity present in polyribosylribitol phosphate was dimethylsulfoxide, the solvent used in the activation reaction with N-hydroxysuccinimidyl ester of β-maleimidopropionic acid. Polyribosylribitol phosphate was purified by filtration using a 2,000 Da cut-off Amicon Ultra-15 to a purity of 99.1 percent and conjugated to tetanus toxoid. The yield of the conjugation reaction with the purified polysaccharide was 30.0 percent ± 1.77 which shows no significant difference with the control which was 33.7 percent ± 3.57 demonstrating that dimethylsulfoxide does not affect the performance of the conjugation reaction(AU)

Introducción: Las úlceras del pie diabético constituyen una de las principales causas de morbilidad y discapacidad, con una importante repercusión por la carga económica de la enfermedad y desde el punto de vista social, por la disminución de la calidad de vida de los pacientes. Objetivo: Evaluar costos y resultado del tratamiento ambulatorio con el Heberprot P® para la atención al paciente con úlcera del pie diabético. Métodos: Se realizó un estudio descriptivo, longitudinal y retrospectivo de 22 pacientes con pie diabético tratados con Heberprot-P®, en la consulta para tales fines del Policlínico Ramón López Peña en el año 2019. Resultados: El 63,6 por ciento de los pacientes que acudieron a consulta presentaron Pie de Riesgo grado cero; la úlcera neuroinfecciosa fue la de mayor incidencia de presentación (94,5 por ciento). Se obtuvo una efectividad del 95,4 por ciento del total de los casos tratados. La mayoría de los tratados necesitaron entre 3-8 bulbos del medicamento, con un promedio de 5 sesiones de tratamiento por paciente, a un costo promedio de $ 3 549,8 /caso tratado. Conclusiones: El tratamiento con el Heberprot P® es altamente efectivo para la cicatrización de las UPD, evitando en gran medida las amputaciones de los miembros inferiores de los pacientes con diabetes mellitus, con una consiguiente disminución del costo económico que tienen estas intervenciones sanitarias para el país(AU)


Introduction: Diabetic foot ulcer is one of the main causes of morbidity and disability, with an important repercussion due to the economic burden of the disease and from the social point of view, due to the decrease in patients' quality of life. Objective: To assess costs and outcomes of outpatient treatment with Heberpro-P® for the care of patients with diabetic foot ulcers. Methods: A descriptive, longitudinal and retrospective study was carried out of 22 patients with diabetic foot treated with Heberprot-P®, in the consultation for such purposes of Ramón López Peña Polyclinic in 2019. Results: 63.6 percent of the patients who came for consultation presented zero risk foot; neuroinfectious ulcer was the one with the highest incidence of presentation (94.5 percent). An effectiveness rate of 95.4 percent of the total of treated cases was obtained. Most of those treated needed three to eight bulbs of the medication, with an average of five treatment sessions per patient, at an average cost of 3549.8 Cuban pesos per treated case. Conclusions: Treatment with Heberprot-P® is highly effective for the healing of diabetic foot ulcers, largely avoiding amputations of lower limbs of patients with diabetes mellitus, with a consequent decrease in the economic cost of these health interventions for the country(AU)

Abatsract Pseudomonas aeruginosa is an important nosocomial pathogen and its clinical importance is mainly related to nosocomial infections. Increased rates of bacterial resistance in recent years has led WHO to publish a global priority list to guide research and discovery of new antibiotics, where P. aeruginosa is among the group of bacteria for which there is a critical level of priority for new drugs to be discovered. In this context, isoeugenol appears as an interesting alternative and the objective of this study was to investigate its action against P. aeruginosa. Isoeugenol presented significant antibacterial activity, with minimum inhibitory concentration (MIC) of 64µg/mL and minimum bactericidal concentration (MBC) of 128µg/mL, and was considered bactericidal against this species. Molecular docking revealed interactions that suggest that isoeugenol may bind to the enzyme Penicillin-Binding Protein 3 and interfere with the bacterial cell wall synthesis process. This study reinforces the antibacterial potential of this compound and emphasizes that more studies are needed in order to better investigate its mechanism of antibacterial action.

Para que os fármacos possam ser comercializados economicamente, a sua escala de produção deve ser aumentada para atender à demanda do mercado. Atualmente, a maior parte dos fármacos são sintetizados em processos batelada que possuem limitações quanto à eficiência de mistura, temperatura e pressão. O uso de microrreatores surge como alternativa na indústria químico-farmacêutica, aumentando a eficiência dos processos de maneira segura. Ferramentas utilizadas no segmento computacional multidisciplinar teórico, como o DFT (Density Functional Theory), podem prever e compreender o comportamento das reações químicas, podendo ter grande utilidade na síntese de novos fármacos economizando tempo, investimento e reduzindo a geração de resíduos. A diabetes mellitus é uma doença de caráter epidêmico, que a cada ano vem aumentando o número de casos. O emprego de fármacos derivados das glitazonas no tratamento de diabetes mellitus tipo 2 é recomendado devido ao excelente controle glicêmico que esta classe de fármacos oferece. Neste trabalho, foi sintetizada a Rosiglitazona, um fármaco derivado das glitazonas, que auxilia no tratamento da diabetes mellitus tipo 2, sendo estudadas duas rotas de síntese distintas, que foram otimizadas com o intuito de maximizar o rendimento de seus intermediários, obtendo a Rosiglitazona com pureza de cerca de 94%. Foi realizada, para os intermediários, aqui denominados, 1R, 2R2 e 3R2 a síntese one-pot e para os intermediários 1R, 2R1 e 3R2 foi realizada a transposição do processo usual em batelada para fluxo contínuo no microrreator, com rendimentos de até 93%. Com o auxílio da química quântica computacional, a reação de síntese do intermediário 1R, foi elucidada teoricamente e determinadas as grandezas termodinâmicas (ΔH‡, ΔG‡ e ΔS‡) no estado de transição, que foram comparadas com os valores experimentais, sendo constatada uma boa concordância, com desvio máximo de 14%


In order for drugs to be commercialized economically, their production scale must be increased to meet market demand. Currently, most drugs are synthesized in batch processes that have limitations in terms of mixing efficiency, temperature and pressure. The use of microreactors appears as an alternative in the chemical-pharmaceutical industry, increasing the efficiency of the synthesis processes in a safe way. Tools used in the theoretical multidisciplinary computational segment, such as DFT (Density Functional Theory), can predict and understand the behavior of chemical reactions, and can be very useful in the synthesis of new drugs, saving time, investment and reducing waste generation. Diabetes mellitus is an epidemic disease that has been increasing the number of cases every year. The use of drugs derived from glitazones in the treatment of type 2 diabetes mellitus is recommended due to the excellent glycemic control that this class of drugs offers. In this work, Rosiglitazone, a drug derived from glitazones, which helps in the treatment of type 2 diabetes mellitus, was synthesized. Two different synthetic routes were studied and optimized in order to maximize the yield of its intermediates, obtaining Rosiglitazone with purity of about 94%. One-pot synthesis was performed to 1R, 2R2 and 3R2 intermediates, and the transposition from the usual batch process to continuous flow in microreactor was performed to 1R, 2R1 and 3R2 intermediates, with yields of up to 93%. With the aid of computational quantum chemistry, the intermediate 1R synthesis reaction was theoretically elucidated and the thermodynamic properties were determined (ΔH‡, ΔG‡ and ΔS‡) in the transition state, which were compared with the experimental results, obtaining good agreement, with a maximum deviation of 14%.

A doença de Chagas é causada pelo Trypanosoma cruzi, e atualmente, acomete entre 6 a 7 milhões de pessoas em todo o mundo. A quimioterapia disponível para seu o tratamento se baseia apenas em dois fármacos, nifurtimox e benznidazol, com mais de 50 anos de descoberto. Estes fármacos apresentam eficácia limitada, pois são pouco efetivos na fase crônica e apresentam alta toxicidade, resultando em efeitos adversos graves. Esse panorama mostra a necessidade de novas abordagens terapêuticas contra essa doença. Nesse sentido, a inibição de vias bioquímicas essencias para o parasita se mostram como uma boa sugestão para identificação de compostos promissores candidatos a novos agentes quimioterápicos. A sirtuína 2 (Sir2) são enzimas reguladoras que participam de mecanismos epigenéticos em tripanossomatídeos, e no T. cruzi possuem um papel fundamental em todos os seus estágios evolutivos, devido a este fato, se apresentam como um alvo promissor na busca por novos fármacos contra a doença de Chagas. Neste sentido propomos a busca de inibidores da Sir2 proteína 1 do T. cruzi (TcSir2rp1) que é geneticamente validada como alvo farmacológico, por meio da estratégia de triagem biológica. Realizou-se a expressão da enzima recombinante por biologia molecular em um sistema de transformação utilizando cepa de Escherichia coli Artic Express (DE3). Foi feita a purificação e a confirmação da obtenção da proteína recombinante se deu por gel SDS-PAGE. Após a obtenção da enzima os parâmetros cinéticos foram determinados por experimentos de fluorimetria. A triagem foi realizada para um conjunto de 82 compostos, previamente sintetizados pelo nosso grupo de pesquisa, como inibidores da TcSir2p1 em dose única de 100 µM. Os ensaios foram realizados em triplicata e em experimentos independentes. Dentre os 82 compostos testados, 20 apresentaram inibições maior que 50% contra a enzima TcSir2rp1, na dose de 100 µM. Dentre estes, se destacaram 3 compostos derivados de chalconas, para os quais foi determinada a potência. O composto 1 foi o que mais potente, apresentando valor de IC50 de 11,65 µM, já os compostos 3 e 5 foram menos potentes (IC50= 38,50 µM e 19,85 µM, respectivamente). Diante dos resultados obtidos, pode-se concluir que a estratégia de triagem biológica é promissora na identificação de inibidores da TcSir2p1 candidatos a agentes anti- T. cruzi


Chagas disease is caused by Trypanosoma cruzi, and currently affects 6 to 7 million people worldwide. The chemotherapy available for its treatment is based on only two drugs, nifurtimox and benznidazole, with more than 50 years of discovery. These drugs have limited efficacy, as they are ineffective in the chronic phase and have high toxicity, resulting in serious adverse effects. This panorama shows the need for new therapeutic approaches against this disease. In this sense, the inhibition of essential biochemical pathways for the parasite proves to be a good suggestion for the identification of promising compounds candidates for new chemotherapeutic agents. Sirtuin 2 (Sir2) are regulatory enzymes that participate in epigenetic mechanisms in trypanosomatids, and in T. cruzi they have a fundamental role in all their evolutionary stages, due to this fact, they present themselves as a promising target in the search for new drugs against Chagas disease. In this sense, we propose the search for inhibitors of Sir2 protein 1 of T. cruzi (TcSir2rp1) which is genetically validated as a pharmacological target, through the biological screening strategy. The expression of the recombinant enzyme was performed by molecular biology in a transformation system using strain of Escherichia coli Artic Express (DE3). Purification was performed and confirmation of obtaining the recombinant protein was performed by SDS-PAGE gel. After obtaining the enzyme, the kinetic parameters were determined by fluorimetry experiments. Screening was performed for a set of 82 compounds, previously synthesized by our research group, as TcSir2p1 inhibitors in a single dose of 100 µM. Assays were performed in triplicate and in independent experiments. Among the 82 compounds tested, 20 showed inhibitions greater than 50% against the enzyme TcSir2rp1, at a dose of 100 µM. Among these, 3 compounds derived from chalcones stood out, for which the potency was determined. Compound 1 was the most potent, with an IC50 value of 11.65 µM, while compounds 3 and 5 were less potent (IC50= 38.50 µM and 19.88 µM, respectively). In view of the results obtained, it can be concluded that the biological screening strategy is promising in the identification of TcSir2p1 inhibitors candidates for anti-T. cruzi agents

Abstract To evaluate the effectiveness of an anticoagulation protocol adapted in a mobile application (appG) for patients using warfarin. This was a cluster randomized controlled clinical trial carried out in basic health centers of Ijui, RS, Brazil, between April and October 2017. The appG was installed on the cell phones of all physicians belonging to the intervention group. Primary outcomes were bleeding and thrombosis, and secondary outcomes were changes in the dose of warfarin, use of new drugs, drug interactions, search for health services, and remaining on the target international normalized ratio. Thirty-three patients belonging to 11 basic health centers were included in this study. From these, 15 patients were in the intervention group which used the appG, and 18 were in the control group. After 6 months, patients in the appG group had fewer bleeding events (7% versus 50%, p-value=0.028) and a lower weekly dose of warfarin (29.3 ± 9.7 mg versus 41.7 ± 12.5 mg, p-value=0.030) when compared to the control group. The anticoagulation protocol adapted in a mobile app reduced bleeding in patients using warfarin.

The first report about antimicrobial resistance was published in the 1940s. And today, the antimicrobial resistance has become a worldwide problem. Because of this problem, there is a need to develop new drugs. That's why we synthesized some novel thiazolidine-4-one derivatives and evaluated their antimicrobial activity. The final compounds were obtained by reacting 2-[(4,5-diphenylthiazol-2-yl)imino]thiazolidin-4-one with some aryl aldehydes. The synthesized compounds were investigated for their antimicrobial activity against four Candida species, five gram-negative and four gram-positive bacterial species. The lead compounds (4a- h) were obtained with a yield of at least 70%. All compounds showed antimicrobial activity. Compound 4f (MIC: 31.25 µg/ml) exhibited more efficacy than the other compounds against C. glabrata (ATCC 24433). Compound 4b (MIC: 62.5 µg/ml) was the most active compound against all bacterial species, particularly K. pneumoniae (NCTC 9633). Whereas, compound 4c (MIC: <31.25 µg/ml) was observed as the most active compound against E. coli (ATCC 25922). In general, all compounds (4a-4h) showed antimicrobial activity against all fungi and bacterial species. Compounds 4b (2,6-dichlorobenzylidene), 4c (2,6-dihydroxybenzylidene), 4f (1H-pyrrol-2- yl)methylene), 4g (4-triflouromethylbenzylidene) and 4h (2,3,4-trimethoxybenzylidene) were determined as the most active compounds

Abstract Innovation is the driving force that is able to create and transform products, processes, and organization in the health system. Innovation in the field of pharmaceutical assistance covers a wide spectrum of aspects, from drug discovery to pharmaceutical care, contributing to the improvement in treatments through novel drugs or methods. This work will present the major characteristics of innovation with special emphasis on aspects pertaining to pharmaceutical assistance. The types and models of innovation, as well as the interaction between academia and industry, will be presented with examples of successful products and methods. In addition, the challenges and perspectives for innovation in pharmaceutical assistance will be discussed with a focus on drug discovery.

El impacto de la COVID-19 en la atención médica dominó las noticias en 2020, incluida la atención del glaucoma. Sin embargo, el reciente cambio de paradigma en la terapia de esta patología hacia una intervención más temprana, un mejor manejo del cumplimiento del paciente y un nuevo enfoque en abordar la patología real, continuó avanzando en este periodo con la aprobación de nuevos medicamentos, productos biológicos y dispositivos, que significaron avances en la atención médica. En este artículo, pretendemos reflexionar sobre los logros obtenidos en el año 2020 en el tratamiento del glaucoma, así como brindar una descripción general de los hallazgos más notables. Se realizó una búsqueda sistemática de la literatura científica de mayor impacto. Se limitó la búsqueda por tipo de diseño (revisiones, series de casos, estudios descriptivos, analíticos y experimentales, metanálisis). No se tuvo en cuenta el idioma de la publicación(AU)


The impact of COVID-19 on medical care was ever-present in the news during the year 2020, and glaucoma care was not an exception. However, the recent paradigm switch in the treatment of this condition towards earlier intervention, better management of patient compliance, and a new approach in addressing the actual disorder, continued to gain ground in this period with the approval of new drugs, biological products and devices leading to the advancement of medical care. The purpose of the study was to reflect on the progress achieved in the treatment of glaucoma in the year 2020, and provide a general description of the most notable findings. A systematic search was conducted in the highest-impact scientific literature. The search was limited to reviews, case series, descriptive, analytical and experimental studies, and meta-analyses. Introducción: Heberprot-P® obtuvo su primer registro sanitario en Cuba en el año 2006, actualmente está aprobado en otros 26 países. Objetivo: Describir el proceso de registro sanitario en México, del medicamento biotecnológico Heberprot-P® para el tratamiento de las úlceras del pie diabético. Métodos: El proceso de registro sanitario de Heberprot-P® siguió las pautas de la reglamentación sanitaria de México sobre la base de la Ley general de salud y el Reglamento de insumos para la salud. Se revisaron además la Farmacopea de los Estados Unidos Mexicanos y las normas oficiales mexicanas en función de cumplir las exigencias para la comercialización de medicamentos en este territorio. Resultados: El proceso de registro se inició en junio de 2017 en México con acciones en función de completar los documentos e informaciones exigidas en el expediente de registro sanitario a presentarse. Entre ellos resaltan las consideraciones del Subcomité de Evaluación de Productos Biotecnológicos y el Comité de Moléculas Nuevas, la evaluación del expediente por un Tercero Autorizado y documentos emitidos por el Centro Nacional de Farmacovigilancia e Instituto Mexicano de la Propiedad Industrial. Se presentó la solicitud del registro sanitario ante Cofepris y esta se aprobó en mayo de 2018. Conclusiones: El trabajo con grupos de expertos permitió a la autoridad mexicana hacer un trabajo más expedito basado en las evidencias de las evaluaciones realizadas que son parte de la información del registro sanitario. Como resultado de este proceso, se otorgó el Registro Sanitario a Heberprot-P® en mayo de 2018 y Cofepris lo reconoció como un medicamento biotecnológico innovador(AU)


Introduction: Heberprot-P® obtained its first Sanitary Registration in Cuba in 2006, and it is currently approved in 26 other countries. Objective: Describe the sanitary registration process in Mexico of the biotechnological drug Heberprot-P® for the treatment of diabetic foot ulcers. Methods: The sanitary registration process of Heberprot-P® followed the guidelines of the sanitary regulations of Mexico on the basis of the General Health Law and the Regulation of Supplies for Health. The Pharmacopoeia of the United Mexican States and the official Mexican standards were also revised in order to comply with the requirements for the marketing of medicines in this territory. Results: The registration process began in June 2017 in Mexico with actions to complete the documents and information required in the sanitary registration file to be submitted. Among them are the considerations of the Sub-committee on the Evaluation of Biotechnological Products and the Committee on New Molecules, the evaluation of the file by an Authorized Third Party and documents issued by the National Center for Pharmacovigilance and the Mexican Institute of Industrial Property. The application for sanitary registration was submitted to Cofepris and this was approved in May 2018. Conclusions: The work with groups of experts allowed the Mexican authority to do a more expeditious work based on the evidence of the evaluations carried out that are part of the information of the sanitary registry. As a result of this process, Heberprot-P® was granted the Sanitary Registry in May 2018 and COFEPRIS recognized it as an innovative biotechnological medicine(AU)

Se realizó un estudio retrospectivo y observacional en pacientes con adenocarcinoma pancreático avanzado, tratados con Nimotuzumab combinado con un esquema de quimioterapia GEMOX, atendidos en el Servicio de Oncología Clínica del Hospital Clínico Quirúrgico Hermanos Ameijeiras entre 2013 y 2019 (n=118), cuyo objetivo fue evaluar la respuesta al tratamiento y la supervivencia global según variables clínicas, histopatológicas y tratamiento. Se utilizó método chi-cuadrado para la asociación de variables. Se evaluó la supervivencia global por el método de Kaplan Meier y se utilizó la prueba de Log-Rank y Breslow para la comparación de las curvas, con valor p<0.05. La tasa de control de la enfermedad fue 44,9. La mediana de supervivencia fue de 13,8 meses (IC95 por ciento:11,7-15,8). Las variables estadísticamente significativas asociadas con mayor respuesta al tratamiento y supervivencia fueron: no hábitos tabáquicos; índice plaquetas-linfocitos por debajo de 200; tumores primarios T2-T3, de localización predominantemente en cabeza pancreática; no presencia de metástasis al diagnóstico y más de seis dosis de quimioterapia o Nimotuzumab. Los pacientes con clasificación de bajo peso y albúmina sérica baja tuvieron peor supervivencia (p<0.05). En 42 pacientes se realizó una segunda línea de quimioterapia y se obtuvo supervivencia de 17,4 meses (IC95 por ciento:13,5-21,4). Las toxicidades clasificadas como grado 3-4 se reportaron en 27 pacientes (22,9 por ciento), las más frecuentemente observadas fueron: neuropatía (14,4 por ciento), neutropenia (10,2 por ciento) y trombopenia (9,3 por ciento). En condiciones de práctica clínica, con el Nimotuzumab combinado con quimioterapia GEMOX se obtuvieron óptimas tasas de control de la enfermedad y supervivencia con buen perfil de seguridad(AU)


A retrospective and observational study was carried out in patients with advanced pancreatic adenocarcinoma, treated with Nimotuzumab combined with a GEMOX chemotherapy scheme, in the Clinical Oncology Service at the Ameijeiras Hospital, between 2013 and 2019 (n =118), whose objective was to evaluate the response to the treatment and overall survival according to clinical, histopathological and treatment variables. The chi-square method was used for the association of variables. Overall survival was evaluated by the Kaplan Meier method and the Log-Rank and Breslow test for the comparison of the curves, with p <0.05. The disease control rate was 44.9. The median survival was 13.8 months (95 percent CI: 11.7-15.8). The statistically significant variables associated with greater response to treatment and survival were: no smoking habits; platelet-lymphocyte index below 200; T2-T3 primary tumors, predominantly located in the pancreatic head; no presence of metastases at diagnosis and greater than six doses of chemotherapy and Nimotuzumab. Patients classified as underweight and low serum albumin had worse survival (p <0.05). Second-line chemotherapy was performed in 42 patients and it was obtained SV of 17.4 months (95 percent CI: 13.5-21.4). Toxicities classified as grade 3-4 were reported in 27 patients (22.9 percent); the most frequently observed were: neuropathy (14.4 percent), neutropenia (10.2 perrcent) and thrombopenia (9.3 percent). Under clinical practice conditions, Nimotuzumab combined with GEMOX chemotherapy obtained optimal disease control and survival rates with a good safety profile(AU)
